Job summary

Cincinnati Children’s Heart Transplant Program is seeking an experienced Heart Transplant Coordinator to support evaluation, transplantation, and lifelong patient management for children with complex heart disease. You will coordinate pre‑transplant evaluations, engage families, and partner with surgeons, physicians, nurses, and social workers.

Primarily weekday responsibilities, some evenings or on‑call may be required.

Qualifications

• Current RN license in Ohio.
• Minimum 5 years of pediatric nursing experience.
• BSN or RN II-aligned degree/experience.
• Strong clinical assessment, critical-thinking, and communication skills.
• Ability to coordinate care across disciplines and families.

Responsibilities

• Serve as a key clinical resource and advocate for patients and families throughout the transplant continuum.
• Lead pre-transplant evaluation coordination and testing arrangements.
• Educate patients and families about transplantation, waiting list, medications, and follow-up.
• Coordinate post-discharge planning and ongoing patient management in clinic and home settings.
• Ensure continuity of care across inpatient, outpatient, and community settings.
• Collaborate with transplant physicians, APPs, nurses, social workers, and financial specialists.
• Maintain accurate transplant records and participate in program quality initiatives.
