Job summary

UVA Health Children's in Charlottesville seeks an Advanced Practice Provider to care for infants, children, and adolescents with complex feeding disorders and enteral nutrition needs. The role provides continuity across inpatient and outpatient settings, allowing the APP to establish relationships during hospitalization and continue care after discharge.

Qualifications

• Graduate of an accredited graduate school of nursing or physician assistant program with a master’s degree.
• Current licensure as an Advance Practice Provider (Nurse Practitioner or Physician Assistant) in Virginia; RN license if NP.
• BLS required; PALS/NRP and ACLS as position requires.

Responsibilities

• Coordinate care across inpatient and ambulatory settings to promote seamless transitions for pediatric patients requiring enteral nutrition.
• Collaborate with interdisciplinary teams to facilitate discharge planning, outpatient scheduling, and care coordination.
• Maintain timely and accurate documentation in the electronic medical record.
• Participate in departmental quality improvement and operational initiatives.
