Responsibilities

• Obtains and documents health histories and performs physical examinations.
• Provides primary, acute and complex care in outpatient settings.
• Orders and interprets diagnostic tests and develops care plans.
• Collaborates with multidisciplinary team to optimize patient care.
• Facilitates transitions between care settings and referrals as needed.
• Documents findings clearly and maintains compliance with clinical pathways.
