Brookside Community Health Center, a Brigham and Women's Hospital site, is seeking a licensed LICSW to join its pediatric-focused social work team. They are dedicated to patient and family-centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate, and ethical problem-solving. As a member of a multidisciplinary health team, they provide short-term clinical therapy, perform comprehensive assessments and develop treatment plans that integrate the medical, social, and resource issues that impact individual patients, families, and at-risk populations. Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community. Brookside also has phlebotomy, financial counseling, and community resource specialists onsite. Each year, more than 12,000 patients from Jamaica Plain, the city of Boston and surrounding areas use these services, producing more than 69,000 visits to Brookside each year. Most of Brookside's patient population are Spanish-speaking and socioeconomically under-resourced.
