Yale New Haven Health is seeking a Pediatric Nurse Coordinator to lead the coordination of patient care within the Pediatric Specialty Centers. The role reports to Nursing leadership and works under the Attending Physician for the specialty, exercising professional judgement to ensure seamless care across the continuum.

The ideal candidate is a Registered Nurse with a BSN preferred, CT RN license, and 2–3 years of RN experience in inpatient or outpatient settings, with pediatric experience

Qualifications

  • RN with Connecticut license; BSN preferred.
  • 2–3 years RN experience in inpatient or outpatient settings.
  • Pediatrics experience or Nurse Coordinator role preferred.

Responsibilities

  • Coordinate all aspects of patient care for the specialty as lead health care team member.
  • Provide patient and family education on conditions, treatments, side effects and outcomes.
  • Collaborate with multidisciplinary team to ensure continuum of care across settings.
  • Participate in data collection for clinical and operational quality improvement.

The Nurse Coordinator is a Registered Nurse, reporting to Nursing leadership within the Pediatric Specialty Centers and under the direction and supervision of the Attending Physician for the specific specialty. The primary role of the nurse coordinator is to ensure seamless coordination of patient care across the continuum. This includes clinical practice, care coordination, education, quality improvement, with significant latitude for exercising professional judgement. The Nurse Coordinator is an integral member of the multidisciplinary team providing developmentally appropriate care to patients and families within the specialty service.

Responsibilities
  • Professional Practice: The Pediatric Nurse Coordinator is the lead member of the health care team coordinating all aspects of patient care for the specific specialty.
  • Education: Provides patient and family education on appropriate medical condition, including signs, symptoms, treatment regimens, side effects, outcomes expected, and potential adverse reactions or outcomes. Including management of condition, promoting optimal level of health.
  • Multi-Disciplinary Collaboration: Functions as a key member of the multi-disciplinary team coordinating all aspects of patient care across the continuum.
  • Quality Improvement: Participates in data collection related to clinical and operational efficiencies.
Qualifications
  • Education: Registered Nurse, Bachelor's Degree in Nursing preferred, Connecticut RN license.
  • Experience: At least 2-3 years RN experience in an inpatient or outpatient setting required. Relevant experience in the specialty area, pediatrics or in the Nurse Coordinator role preferred.
  • License: Licensed as a Registered Nurse from the State of Connecticut.
  • Special Skills: Excellent interpersonal and organizational skills. Ability to assert oneself, advocate for patients, and work independently with minimal supervision. Ability to manage multiple priorities. Computer experience and knowledge of word processing and data entry systems. Must be able to communicate effectively with all members of the healthcare team, patients and families. Must value and execute \"patient-centered model of care\".
  • Physical Demand: Must be flexible to meet the demands of the specialty, outpatient clinic session, specialty and patient/family needs.
