Job summary

Medical University of South Carolina seeks a Social Worker (MSW) to coordinate discharge planning in collaboration with the health care team to achieve targeted patient outcomes. You will assess patients and caregivers during hospitalization and follow-up, identifying post-discharge risk factors and applying interventions to reduce risk.

The MSW serves as a bridge to outpatient providers to address questions and ensure continuity of care.

Qualifications

• Master's degree in social work from an accredited program.
• MSW licensure by the SC Board of Social Work Examiners.
• Case management certification is preferred.

Responsibilities

• Coordinate the discharge process with the health care team to support successful transitions.
• Assess patients and caregivers during hospitalization and follow-up as needed.
• Identify post-discharge risk factors and apply interventions to lower risk.
• Bridge discharge planning to outpatient providers to address questions and coordinate care.
