Job summary

Nicklaus Children's Hospital seeks a Nurse Navigator for the Diabetes Program to coordinate care from ED to inpatient units and after discharge. The role emphasizes patient education, rapid access, and barrier elimination to timely treatment.

You will collaborate with endocrinology teams, nursing leadership, and community partners to ensure smooth transitions and evidence-based practice. This full-time Days position supports families and pediatric patients throughout the care continuum.

Qualifications

• Bachelor's Degree BSN from an accredited program required
• RN licensure in Florida or multi-state compact required
• BLS CPR certification required
• PALS certification required
• 3–5 years pediatric clinical experience preferred
• 1–3 years diabetes educator role preferred

Responsibilities

• Coordinate and communicate the patient care plan with physicians and staff
• Develop and enhance clinical processes for efficient inpatient/outpatient transitions
• Collaborate with community and referring physicians for seamless care
• Provide patient/family education and resources about diabetes management
• Lead education efforts for residents, nursing staff, and educators
