Job summary

University of Vermont Medical Center is establishing Complex Care at the Golisano Children’s Hospital by fall 2026. The nurse on the team coordinates care for children with complex medical needs, collaborates with interdisciplinary teams, and mentors colleagues to improve outcomes.

The role emphasizes pulling together resources, monitoring health care utilization, and addressing barriers to care while supporting families and communities in Vermont.

Qualifications

• Current RN licensure or compact licensure recognized by the State of Vermont required.
• ADN required, BSN preferred.
• Appropriate experience in specific clinical area. Varies by unit.

Responsibilities

• Coordinates the care and services of selected patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources.
• Applies nursing scope of practice and clinical knowledge to care for children with complex medical conditions.
• Actively participates in mentorship, training, and process improvement within the team.
• Identify and intervene to address barriers to treatment, health, wellness, prevention and outcomes.
