University of Vermont Health - UVM Medical Center seeks a nurse team specialist for Complex Care at the Golisano Children's Hospital, launching fall 2026.

You will coordinate care for children with medical complexity across the continuum, apply nursing scope, and collaborate with interdisciplinary teams to achieve optimal outcomes. Mentorship and process improvement are key components.

Requires VT RN licensure, ADN, 2+ years pediatric experience; BSN preferred; Epic/VITL experience a plus.

Qualifications

• Current RN licensure in Vermont required.
• ADN degree required; BSN preferred.
• Minimum 2 years of clinical experience; pediatric complex conditions preferred.
• Experience as care coordinator/case manager preferred; Epic and VITL experience preferred.

Responsibilities

• Coordinate care for children with medical complexity across the continuum.
• Apply nursing scope of practice and collaborate with interdisciplinary teams.
• Address barriers to treatment and participate in mentorship and process improvement.
• Serve as liaison between patient, healthcare teams, and community resources.
