Job summary

OU Health is seeking a Pediatric Care Management Social Worker PRN - Inpatient (Weekends) to coordinate care for high-risk pediatric patients within the inpatient setting. You will collaborate with the care team, families, and community services to ensure timely access and continuity across settings.

Under Social Worker II supervision, you will assess needs, develop care plans, provide counseling, and support transitions while engaging in ongoing education and leadership activities.

Qualifications

• Master of Social Work from an accredited institution required.
• Current LMSW or under supervision for LCSW; LMSW/LCSW eligibility required.
• Basic Life Support certification from the American Heart Association required.

Responsibilities

• Assess patients using medical data, interviews, and care teams.
• Provide social services and supervise care plan interventions.
• Educate patients and families about diseases, treatments, and resources.
• Monitor progress and intercede with crisis counseling when needed.
• Serve as liaison between patients, families, and clinical staff.
• Lead interdisciplinary team meetings and rounds.
• Precept and train intern staff and students.
