Qualifications

• AAS or BA in Social Work, Psychology, or related Human Services field is required.
• Related internships will be considered.

Responsibilities

• Coordinate daily care for child/adolescent up to age 21.
• Conduct comprehensive health assessments using NYSDOH tools (CANS-NY & UAS).
• Collaborate with Social Work, care coordinators, medical staff, nursing and ancillary services to ensure coordinated care.
• Assist families to access care in the most appropriate setting and set/monitor goals.

Education

AAS - Associate of Applied Science Social Work, Psychology or related Human Services field required.

BA - Bachelor of Arts Social Work, Psychology or related Human Services field required.
