Job summary

Cleveland Clinic Health System is seeking an RN Care Coordinator in Pediatric Cardiology Fetal Care on the Main Campus. The role involves coordinating care for high‑risk patients, conducting comprehensive assessments, and guiding families through disease management and treatment plans.

You will work with a multidisciplinary team, focus on reducing readmissions, and support patients across the continuum of care, Monday through Friday. This position offers growth within a world‑class health system.

Qualifications

• RN required and licensed in the state.
• BLS certification required (AHA or Red Cross).
• 3–5 years of nursing experience.
• Cancer-specific certification or ongoing qualifying education if working in oncology settings.

Responsibilities

• Coordinate care and disease management longitudinally for high‑risk patients.
• Assess comprehensive clinical needs across medical, behavioral, social domains.
• Provide education and coaching to patients and families; monitor progress.
• Identify barriers and coordinate transitions of care across settings.
• Engage multidisciplinary teams to develop goals and plan interventions.
• Conduct targeted outreach to patients with ongoing needs.
• Act as liaison and advocate for patients and families across the care continuum.
