Qualifications

• State licensure as a Licensed Advanced Social Worker or similar required.
• Skills in individual and group counseling, evaluation, and care plan development.
• BLS (CPR/AED) certification and a valid driver's license needed.

Responsibilities

• Provide individual and group counseling services.
• Collect psycho-social data and assist with medical care management.
• Develop detailed care plans and link clients to community resources.
