Job summary

Valley Health Partners Community Health Center in PA seeks a Behavioral Health Specialist for the Pediatric Southside Clinic. The role completes assessments, develops treatment goals, and facilitates referrals within the center and the local community to address patient mental health needs.

Requires a Master’s Degree in Counseling, Social Work, or Psychology, and 2 years of clinical experience. LCSW or LPC licensure upon hire is expected.

Qualifications

• Requires a Master’s degree in Counseling, Social Work, or psychology.
• Two years of clinical counseling or social work experience.
• Licensure as LCSW or LPC upon hire is required.

Responsibilities

• Assesses behavioral health needs and barriers to care.
• Develops and implements treatment goals.
• Facilitates internal and community referrals/resources.
• Completes medical record documentation timely and accurately.
• Collaborates with internal/external teams to coordinate care.
