• Identification and stratification of the patient population.
• Identify and stratify the targeted population according to behavioral risk and needs.
• Develop, implement and update patient care plans.
• Manage a panel of patients including care management plans and goals in depression, anxiety, ADHD, and related areas.
• Provide counseling for patients with chronic illness.
• Develop care plans with quarterly updates.
• Review patient panel and document care activities monthly.
• Coordinate patient care with PCP and healthcare teams.
• Assist patients with self-management through education and telephonic engagement.
• Assist patients by conducting ongoing assessments telephonically and coordinating care conferences.
• Navigate patients through the healthcare system.
• Engage patients to manage health using CBT, motivational interviewing and mindfulness-based interventions.
• Encourage adherence to care plans and collaborate with care teams.
• Attend trainings and practice team meetings.
