Qualifications

  • Current CPR certification.
  • Licensed driver with automobile insured.

Responsibilities

  • Assumes responsibility for professional nursing practice per the state Nurse Practice Act and standards for home health nursing.
  • Performs physical assessments of clients; may complete admissions and reassessments as directed by Clinical Supervisor.
  • Supervises LPN/LVNs and HHAs under the Clinical Supervisor.
  • Reports changes in client condition promptly to the physician and care team.
  • Participates in development of the client care plan.
  • Maintains confidentiality and upholds safety and infection control practices.
  • Delegates tasks based on scope of practice and circumstances.
  • Performs other duties as deemed necessary.

Job description:

Provides services to clients in accordance with the state Nurse Practice Act, Agency policy, and accepted professional standards of practice. The Registered Nurse (RN) provides nursing services utilizing a comprehensive base of nursing theory under the direction of a medically approved care plan.

REPORTS TO: Clinical Supervisor

PROVIDES SUPERVISION TO: NA – No supervision responsibilities

Qualifications
  • Graduate of an accredited school of professional nursing. BSN preferred.
  • Current license as a Registered Nurse in the state of operation.
  • Demonstrated strong communication and interpersonal skills.
  • Current CPR certification.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements.
Essential Functions/Areas Of Accountability
  • Assumes responsibility and accountability for the practice of professional nursing in accordance with the state Nurse Practice Act and standards for home health nursing.
  • Performs physical assessments of clients. May complete comprehensive assessments for client admissions and reassessments at the discretion of the Clinical Supervisor.
  • Supervises Licensed Practical/Vocational Nurses (LPN/LVNs) and/or Home Health Aides (HHAs) at the discretion of the Clinical Supervisor.
  • Reports changes in client condition promptly to the medical practitioner overseeing care as well as to other members of the client’s care team.
  • Participates in the development of the client care plan.
  • Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice.
  • Practices nursing with respect for individual, cultural, and spiritual differences.
  • Follows Agency policies and procedures as well as payor and accreditation requirements.
  • Initiates actions to improve the quality of client care and to prevent negative client outcomes. Continually promotes safety and infection prevention and control practices.
  • Maintains confidentiality in all aspects of the job.
  • Participates in ongoing, professional self-development.
  • Delegates tasks to Licensed Practical/Vocational Nurses (LPN/LVNs) and/or Home Health Aides (HHAs) based on their legal scope of practice and where circumstances require.
  • Performs other duties as deemed necessary.
