VIVA Pediatrics is a home health services company specializing in private duty nursing, therapy, and skilled nursing visits for infants and children. We are committed to providing each patient with an exceptional level of care.

Position: PDN RN/LVN - 1 y/o (GB/Seizures/Oxygen) Rockdale, TX | Mon-Fri (6A-6P)

• Assess home health patients to identify physical, psycho-social, and environmental needs, documented through clinical records, case conferences, team reports, and evaluations.
• Develop, implement, and document the plan of care to ensure quality and continuity of care.
• Provide care following infection control measures to protect staff and patients, complying with OSHA standards and all relevant regulations.
• Adhere to VIVA Pediatrics policies and promote the agency’s philosophy.
• Communicate effectively with patients, families, team members, and other healthcare professionals, as evidenced by clinical notes and communication records.
• Notify the supervising nurse immediately of any changes in the patient’s condition.
• Provide health education to patients and/or caregivers.
• Timely document any interim changes to the care plan.

Requirements:

• Minimum 6 months pediatric home health experience or 1 year of RN/LVN work experience.
• CPR Certification.
• Valid Driver's License.
