Qualifications

• High school diploma or equivalent.
• Vermont Nursing Assistant licensure is required.
• Current BLS Healthcare Provider status.
• OSHA Category I - tasks with exposure to blood or body fluids require Universal Precautions.

Responsibilities

• Provides direct patient care under the supervision of a licensed nurse.
• Assists with activities of daily living (bathing, dressing, grooming, toileting, feeding).
• Obtains and documents routine vital signs, weights, intake and output.
• Assists with transfers, ambulation, positioning, and safe patient handling.
• Reports changes in patient condition to the licensed nurse.
• Maintains a safe, clean patient environment and follows infection prevention practices.
• Assists with admissions, transfers, and discharges; prepares rooms and ensures comfort.
• Responds to patient requests and call lights; promotes a positive patient experience.
• Assists with specimen collection and delegated clinical tasks within LNA scope.
• Documents care accurately and timely per policy.
• Protects patient privacy and rights per laws and policies.
• Collaborates with nurses, physicians, therapists, and other team members.
• Ensures patient safety by identifying hazards and following fall prevention protocols.
