PCN Nurse Triage RN (CLIN NURSE 2)


Job Summary

The CN identifies and implements nursing interventions and evaluates results for a wide variety of patient populations, directs and acts as resource to other members of the nursing team as assigned and promotes compliance and support for policies. The primary responsibilities for this position consist of providing total, comprehensive, continuous and patient centered care which includes: assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive nursing plan of care; delivering direct patient care; ensuring patient satisfaction; training and delegating duties; and adhering to University policies and procedures as well as federal, state and local regulations.

Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care.

Provides direct patient care to a diversified patient population. Applies knowledge, experience and judgment to determine the importance of a situation, set priorities, and use abstract thought to evaluate clinical situations. Participates in patient and staff education. Demonstrates leadership skills. Demonstrates accountability for own practices as defined by the Nurse Practice Act.

Qualifications



Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position
  • Valid California Registered Nurse License without restriction
  • American Heart Association (AHA) Basic Life Support (BLS) or UC Davis Health CPR
  • Graduation from an accredited school of nursing.
  • At least five (5) years of recent RN experience
  • Experience and advanced understanding of the medical management of pediatric and/or adult patient populations with acute and chronic illness.
  • Experience providing nursing care for patients being co-managed by multiple providers related to comorbidities.
  • Experience providing patient education to pediatric and/or adult patient populations with acute and chronic illnesses.
  • Experience providing nursing care for acute and chronic conditions in pediatric and/or adult patient populations such as fever, cough, respiratory distress, respiratory infections, infectious diseases, abscesses, rash, bleeding, constipation, acute and chronic wounds, injuries, pain, fractures, dizziness, vomiting, diarrhea, UTI, Congestive Heart Failure, COPD, hypertension, asthma, stroke, chest pain, MI, Diabetes Mellitus, arthritis, anxiety, and depression.
  • Ability to read, write, speak, and understand the English language.
  • Ability to utilize medical terminology appropriately.
  • Knowledge of current trends in nursing.

  • Knowledge of acute and chronic conditions in pediatric and/or adult patient populations such as fever, cough, respiratory distress, respiratory infections, infectious diseases, abscesses, rash, bleeding, constipation, acute and chronic wounds, injuries, pain, fractures, dizziness, vomiting, diarrhea, UTI, Congestive Heart Failure, COPD, hypertension, asthma, stroke, chest pain, MI, Diabetes Mellitus, arthritis, anxiety, and depression.
  • Mastery of Relational Competencies.

Department Overview

Nurse Triage provides comprehensive telephonic advice to patients from 14 Primary Care UC Davis clinics, 3 of which are hospital-based. Within Nurse Triage, Registered Nurses provide services to patients ranging from newborn to geriatric. We handle both advice calls and emergent calls and determine the disposition for each patients' care, whether it be home care, office visit, or ED. The call volume daily ranges from 400-500 calls. Nurse Triage also houses the MyChart Message Team where RNs perform initial intake of patient MyChart messages for 11 primary care clinics. The nurses handle an average of 1,100 messages per day. Additionally, the department support the UC Davis MD Consult line, UC Davis Express Care, and Urgent Care at Home referrals.

Work Environment
