Job summary

Divine Home Health Care, LLC is seeking compassionate home health aides to support clients in Minnesota and surrounding areas. You will assist with daily living activities, monitor health, and report changes to the supervisor.

Responsibilities include bathing, grooming, mobility, meal prep, light housework, medication reminders, and accompanying clients to appointments. Documentation and EVV time records must be maintained accurately.

Qualifications

• Assist with activities of daily living (ADLs) such as bathing, grooming, personal hygiene, dressing, toileting.
• Support mobility and transfers for clients.
• Provide companionship and emotional support.
• Assist with meal preparation and feeding.
• Light housekeeping and cleaning.
• Reminders for medications (no administration unless authorized).
• Accompany clients to appointments or errands as needed.
• Monitor client condition and report changes to supervisor.
• Maintain accurate documentation and time records (EVV if applicable).
