Job summary

Charter Oak Home Care in West Hartford is seeking a caring caregiver to support a woman Monday through Friday from 1pm to 5pm (flexible hours possible with multiple client openings). The client is a fall risk and will require help with transferring, showering/toileting, housekeeping tasks, and overall companionship.

Pay ranges from $18 to $23 per hour. We welcome experienced caregivers (PCA, HHA, CNA) who are reliable and passionate about helping older adults remain independent at home.

Qualifications

• Compassion and empathy are a must.
• Alzheimer's or Dementia experience is a plus.
• Ability to use good judgement and remain calm.
• Reliable and on time.
• Proof of eligibility to work in the U.S.
• Background check and references required.

Responsibilities

• Deliver exceptional in-home care to the client.
• Execute personalized weekly care plans and schedules.
• Provide companionship and social/emotional support.
• Assist with personal care: hygiene, mobility, light housekeeping, meals.
• Ensure safety, order, and client enjoyment.
• Provide transportation to appointments as needed.
• Document tasks and report changes in the client’s condition.

We are looking for a caregiver to work Monday-Friday 1pm-5pm with a woman in West Hartford. This client is a fall risk and will need help with transferring, showering/toileting, housekeeping tasks, and overall companionship.

Pay $18-$23/hour

We are seeking experienced caregivers (PCA, HHA, CNA) who are reliable, have a heart for service, and a passion to care for the older adults to join our team. If you are looking for an opportunity to design a flexible schedule, we have several current openings with clients. The hours vary from 4-12 hour shifts on all days of the week and our caregivers can design either a part time or full time schedule.

What You Do

You have the ultimate responsibility to help implement care plans that nurture older adults in mind and body. You will provide personalized care to clients assisting in scheduled activities as well as overall care management. Our hope is that you will treat your clients like family, going above and beyond to enhance their quality of life.

Responsibilities

• Deliver exceptional care to dedicated client in their home
• Help execute comprehensive personalized weekly schedules and care plans
• Supply personal companionship, remain actively engaged with each client by providing ongoing social & emotional support
• Assist with personal care activities to include personal hygiene, mobility assistance, light housekeeping, feeding and meal preparation
• Keep order, safety and client enjoyment top of mind
• Provide transportation to and from appointments as needed

• Administrative functions such as documentation of completion of tasks and report changes in the client’s condition
