Job summary

Changing Lives Family Care seeks a dedicated Personal Care Aide (PCA) to provide person-centered assistance in clients’ homes and community settings. You will support with ADLs, personal care, and light housekeeping while following each plan of care and maintaining professional standards.

PERSONAL CARE AIDE (PCA) JOB DESCRIPTION

Position Title: Personal Care Aide (PCA)
Department: Personal Care/Home Care Services
Reports To: Registered Nurse (RN), Supervisor, or Administrator
Employment Status: Full-Time / Part-Time / PRN
Position Type: Direct Care

POSITION SUMMARY

The Personal Care Aide (PCA) provides person-centered assistance and support to individuals in their homes and community settings. The PCA assists individuals with activities of daily living (ADLs), personal care, household activities, and other authorized tasks according to the individual's plan of care.

The PCA promotes each individual's safety, dignity, privacy, independence, preferences, and quality of life while maintaining professional boundaries and complying with On Point Home Care Inc. policies and applicable requirements.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The PCA will:

• Assist with bathing, grooming, dressing, oral hygiene, and other personal-care needs.
• Assist with toileting, continence care, and personal hygiene.
• Assist with transfers, positioning, ambulation, and mobility as authorized.
• Assist with eating and meal preparation according to the individual's needs and plan of care.
• Provide assistance with light housekeeping related to the individual's health and safety.
• Assist with laundry, changing bed linens, and maintaining a clean living environment.
• Provide reminders for medications when permitted; PCA will not administer medications unless specifically authorized and qualified under applicable requirements.
• Observe the individual's physical, behavioral, and functional condition and promptly report changes to the appropriate supervisor.
• Follow the individual's established plan of care and complete only authorized duties.
• Encourage the individual to participate in activities and maintain the highest practical level of independence.
• Provide companionship and appropriate social interaction.
• Follow infection-control practices, including proper hand hygiene and use of personal protective equipment when required.
• Immediately report suspected abuse, neglect, exploitation, injuries, emergencies, or other significant concerns according to agency procedures.
• Follow emergency and safety procedures.
• Maintain confidentiality and protect the individual's health and personal information.
• Complete accurate service documentation, timesheets, electronic visit verification (EVV), and other required records.
• Report missed visits, late arrivals, schedule changes, or inability to complete assigned duties immediately.
• Attend required orientation, training, competency evaluations, supervision, and in-service education.
• Maintain professional behavior, appearance, communication, and boundaries at all times.
• Perform other authorized duties consistent with the individual's plan of care and the PCA's training and scope of responsibility.

PCA SHALL NOT

The PCA shall not independently perform skilled nursing procedures or other duties outside the PCA's training, authorization, or assigned plan of care.

MINIMUM QUALIFICATIONS

The PCA must:

• Meet applicable agency and regulatory requirements for employment as a Personal Care Aide.
• Successfully complete required PCA training and competency requirements.
• Demonstrate the ability to read, understand, and follow written and verbal instructions.

PHYSICAL REQUIREMENTS

The employee must be physically capable of performing assigned personal-care duties, which may include standing, walking, bending, reaching, assisting with transfers and mobility, and lifting or supporting individuals within established safety procedures and assigned job requirements.

CONFIDENTIALITY

The PCA must maintain strict confidentiality regarding all individual, family, employee, and agency information and comply with HIPAA and On Point Home Care Inc. confidentiality policies.

PERFORMANCE EXPECTATIONS

The PCA is expected to provide services in a safe, respectful, reliable, person-centered, and professional manner. Failure to follow the plan of care, document services accurately, maintain confidentiality, report significant changes, or comply with agency policies may result in corrective action.
