Job summary

Presbyterian Healthcare Services in New Mexico is seeking an LPN to join the ambulatory care team. You will provide patient care in collaboration with care managers and medical providers as part of the primary care team.

Responsibilities include administering medications and treatments, preparing patients for visits, and supporting providers with procedures while maintaining efficient patient flow and adherence to standards of care.

Qualifications

  • High School Diploma or GED.
  • Current Licensed Practical Nurse - NM.
  • BLS required.

Responsibilities

  • Administer prescribed medications and treatments, as ordered by provider/Standard of Care Protocol.
  • Provides patient care in the ambulatory care setting according to established standards in collaboration with Providers, Registered Nurses and Medical Assistants as members of the care team.
  • Provides primary support to the provider by proactively anticipating the provider needs, preparing the patient for the visit, maintaining efficient patient flow, and assisting with treatments and procedures.
  • Participates on interdisciplinary team to meet patient care and clinic needs, working cooperatively with others to accomplish required work in a timely and efficient manner.
  • Assists providers with procedures.
  • Assists with patient education, within scope of practice by Standard of Care-Education Protocol.
  • Assists with visit planning, within Health Maintenance Guidelines.
  • Assist with management, in collaboration with Clinical Manager.
  • Documents in electronic medical record within scope of practice.

  • Assists Certified Medical Assistants with patient preparation/rooming patients, as directed by visit plan.

  • Applies Compassionate Connected Care principles and incorporates into care interactions

  • Interacts effectively with customers, providing instructions and information, both written and verbal, as appropriate.

  • Maintains established practice policies and procedures, objectives, quality program, safety, environment, and infection control standards.

  • Seeks help, communicates, and cooperates with Medical Provider and team appropriately.

  • Meets assigned productivity targets

  • Performs other functions as required.
