Job summary

Legacy Health is seeking a Transportation Coordinator to review and coordinate non-emergent medical transportation for patients both leaving the hospital and within the system in Portland, OR. You will work with internal teams and external partners to prioritize patient needs and ensure timely, safe transport.

In a 24/7 lean department, you will identify opportunities to streamline processes, manage data in Epic, and support exceptional patient care through proactive problem-solving and strong

Qualifications

  • High School diploma or equivalent required.
  • Two years of healthcare work environment experience required.
  • Associate’s degree in a related field preferred.
  • Strong organizational and communication skills.
  • Experience with Electronic Medical Records (Epic) is a plus.

  • Review and coordinate non-emergent medical transportation requests for patients leaving the hospital and within the hospital system.
  • Coordinate transportation logistics with internal and external stakeholders to prioritize patient needs.
  • Support 24/7 operations by identifying process improvements and increasing efficiency.
  • Proactively solve problems to ensure high-quality patient transportation services.

Responsibilities
  • Responsible for reviewing and coordinating non-emergent medical transportation requests for patients leaving the hospital as well as acute patient transport within the hospital system.
  • Works closely with various stakeholders (internal and external) to coordinate transportation logistics and prioritize patient needs.
  • Operating within our 24/7 lean department, you will be an integral part of our continuous improvement efforts, identifying opportunities to streamline transportation processes and enhance operational efficiency.
  • Your proactive approach to problem-solving and your commitment to excellence will contribute to our overarching goal of delivering exceptional patient care.
Qualifications
  • High School diploma or equivalent required.
  • Associate's degree in healthcare administration, business, behavioral health, social services or a related field preferred.
  • Two years of healthcare work environment experience required.
  • Strong organizational skills
  • Knowledge of community resources and the social service delivery system
  • Excellent organization, oral, and written communication skills for effective interaction with various stakeholders
  • Ability to foster communication and build relationships among the treatment team
  • Excellent time management skills
  • Computer literate and ability to utilize Microsoft programs and Electronic Medical Records System (Epic)
  • Data management

  • Investigate and create new transportation pathways.
Pay Range

USD $25.83 - USD $36.93 /Hr.
