Responsibilities

• Serve as liaison between patient families/caregivers and care team for discharge communication.
• Identify and mitigate barriers to discharge before discharge day.
• Verify equipment orders to ensure safe discharge.
• Communicate follow-up and specialty appointments to patients/caregivers.
• Confirm prescriptions are sent to patient’s pharmacy.
• Notify nurse when patient is ready for discharge and ensure orders, meds, and transportation are in place.
• Coordinate post-discharge communication with Case Managers for high-risk patients.
