Coordinate and schedule patient visits using our electronic medical record system
Partner with nurses, therapists, and the interdisciplinary team to ensure seamless patient care
Monitor reports and proactively follow up on incomplete or untimely visits
Collaborate with field clinicians and leadership to optimize productivity, coverage, and scheduling efficiency
Utilize analytics to reduce missed visits and support balanced caseload distribution (RN/LVN, Therapist/Assistant, FT/PRN)
Communicate clearly and professionally with patients, staff, referral partners, and leadership
Support quality initiatives, including participation in QAPI programs
Promote VitalCaring’s philosophy and standards of care in every interaction
At least one year of general office experience (healthcare or hospice)
High school diploma or equivalent (18+ years of age)
Strong organizational skills and attention to detail
Ability to multitask in a dynamic clinical environment
If clinical or making patient visits: current state license (if applicable), valid driver’s license, auto insurance, and CPR certification
