Job description

The Patient Safety Associate (PSA) provides 1:1 safety observation and basic direct patient care under the supervision of the RN. Responsibilities include, but are not limited to, visual observation, verbal re-direction, safety checks, environment checks, care and comfort, vital signs recording, personal hygiene, activities of daily living (ADLs), mobility, and fall prevention. The PSA acts as a liaison between the patient and nurse to report concerns or changes, offers customer service to patients, families, visitors, and documents patient care when applicable.

Essential Functions
  1. Provide 1:1 safety observation for high-risk patients, including those with suicidal tendencies, psychiatric conditions, medical or surgical needs, dementia, delirium, traumatic brain injury, substance withdrawal, or other diagnoses posing potential harm.
  2. Use appropriate verbal interventions and de-escalation techniques; explain care to patients as necessary without providing counseling or opinions.
  3. Maintain a safe and conducive environment for recovery, including safety checks, room inspections for ligature points and contraband, cleanliness, linen changes, and waste disposal.
  4. Under RN or licensed practitioner supervision, deliver safe, direct care tailored to patient age and needs, including hygiene, feeding, transportation, toileting, ambulation, and safety measures, in accordance with policies and care plans. Report any incidents immediately.
  5. Identify personal learning needs, demonstrate commitment to professional development, and adhere to clinical and administrative directives. Communicate effectively with healthcare team members and resolve or report interaction issues. Attend staff meetings, participate in QA/CQI activities, and complete documentation accurately within specified time frames.
  6. Document patient care and activities thoroughly using established communication systems, ensuring 100% completion for assigned patients and timely reporting of observations to supervising personnel.
  7. Perform additional duties or participate in projects to support the department’s mission, assisting team members and accepting assignments as needed.
Qualifications
  • Required Education: High school diploma or GED
  • Preferred Education: Certified Nursing Assistant (CNA) or Medical Assistant program
  • Licensure/Certification Required: Current AHA BLS Healthcare Provider certification
  • Preferred Licensure/Certification: CNA or Medical Assistant
  • Additional Skills and Experience: One year of experience as a CNA or Medical Assistant, or in a healthcare-related field with direct patient care
