Responsibilities

  • Observes and monitors patients identified to be at risk for self-injury, fall, and/or otherwise behaviorally impaired.
  • Notifies the patient’s nurse when patient displays actions or behaviors suggesting the patient will cause harm to themselves or others or when the patient is unable to utilize the call light and is requesting assistance.
  • Attends to patients activities of daily living and will obtain and report vital signs and intake and output to the primary nurse.
  • Documents patient behavior according to District policy 910-032 during their assigned shift using all required forms or reporting systems as appropriate.

Job Summary:
