What to Expect (Job Responsibilities):
- Act as a liaison between patients and community resources, ensuring timely access to housing assistance, Medicaid enrollment, and food resources
- Collaborate with care managers and healthcare providers to address patients' social, financial, and transportation needs
- Assist patients with the process of applying for Medicaid and other public assistance programs
- Maintain accurate records of patient interactions and resource referrals in compliance with HIPAA and company policies
- Educate patients about available resources and provide follow-up support to ensure successful implementation of services

What is Required (Qualifications):
- High School Diploma or equivalent required
- At least 2 years of experience in case management, social work, customer service, or a related field
- Strong understanding of social determinants of health and their impact on patient outcomes
- Excellent interpersonal skills and ability to communicate effectively with diverse populations
- Proficient with technology, including Google tools and various digital platforms

- College degree in Social Work, Healthcare Administration, or a related field
- Experience with public assistance programs, including Medicaid and housing resources
