Job summary

Hackensack Meridian Health is seeking a Patient Placement Coordinator in New Jersey to support efficient patient flow and safety. The role involves collaborating with capacity managers, nursing leadership, infection control, and transport teams to optimize admissions, discharges, and transfers.

Ideal candidates will have a high school diploma or GED and at least 2 years of hospital experience, plus strong computer skills (Outlook, Word, Excel).

Qualifications

• High School diploma or GED is required.
• Knowledge and experience with computers, OS usage, Outlook, Word, Excel and basic keyboarding skills.
• Minimum of 2 years in a medical/acute hospital setting is required.

Responsibilities

• Collaborates with Capacity Manager(s), nursing supervisors/Directors, infection control staff, ED leadership, EVS & Transport leadership to ensure efficient execution of patient placement decisions to ensure patient safety.
• Maintains ongoing communication with Capacity Managers & aforementioned staff to maintain continual patient flow and ensure essential personnel are informed
• Utilizes appropriate tools and technology to monitor and promote continuous patient movement.
• Ensures accurate admission, discharge, and transfer data and all patient flow measures and statistics as defined by Capacity Management team
• Demonstrates expertise in working with required computer applications, phone systems and patient tracking tools
• Interacts with all required stakeholders (nursing, physicians, EVS, transport, leaders and team members, etc) utilizing appropriate communication skills and tools. Performs daily work tasks maintaining extraordinary Customer Service at all times
• Other duties and/or projects as assigned
• Adheres to HMH organizational competencies and standards of behavior

Starting Minimum Rate

Minimum rate of $20.64 Hourly
