Job summary

NewYork-Presbyterian in Bronxville is seeking a Part-Time Patient Navigator for our Sleep Center. The role serves as the primary care coordinator for patients, working with clinical staff to arrange appointments, collect records, and communicate across teams.

You will interact with patients in person, by phone or email during day shifts (3 days per week: Mon, Wed, Fri) and must live within commuting distance to NYP. A health care background and bilingual English/Spanish are preferred.

Qualifications

• Bachelor's degree preferred; health care experience is a plus.
• High School Diploma or GED required.
• Bilingual English/Spanish is preferred.

Responsibilities

• Primary care coordination for Sleep Center patients.
• Coordinate appointments and gather medical records, insurance and financial information.
• Serve as liaison between patient and medical team; handle inquiries by in person, phone or email.

The Patient Navigator functions as the primary care coordinator for patients and collaborates with key clinical staff to ensure comprehensive care and treatment. Coordinates appointments, provides logistical support, acts as a liaison between the patient and medical team, directs patient inquires to appropriate staff, and facilitates collection of medical records, insurance and financial information. Position includes direct contact and communication with patient, in person, on telephone or via e-mail.

This is a Part- Time opportunity is to cover the day shift, 3 days per week, M, W, F 8:00am - 4:00pm at NYP Westchester. Located in Bronxville, right across from the Metro North Train Station.

Preferred Criteria

• Bachelor's Degree from accredited college or university
• Experience in health care setting working directly with patients
• For Pediatrics, experience in a healthcare setting working direction with pediatric patients
• Bilingual (English/Spanish)

Required Criteria

• High School Diploma or GED
• Basic computer skills (Microsoft Word, Excel)
• Excellent oral and written communication skills
• Ability to manage high stress, chronically ill population with complicated psycho-social issues
• Diverse and culturally sensitive customer service skills

Salary Range:

$21.35 - $34.15/Hourly
