Major Responsibilities
  1. As an integrated member of the primary medical care team, assess patient needs for social services, provide referrals, and facilitate patient access to care.
  2. Provide education, information, and referrals to specialized intervention programs, advocating for patient access to these services.
  3. Function as a navigator among service providers and community resources, maintaining ongoing communication with the clinical team.
  4. Identify patients who are uninsured or need medication coverage, and assist with medical and social service history.
  5. Attend periodic team meetings and staff education meetings to maintain current knowledge applicable to the program.
  6. Adhere to regulatory policies and procedures relative to the program.
Position Qualifications

License/Certification/Education:

  1. Required: High School diploma or GED.
  2. Preferred: Bachelor’s Degree in Social Work, Human Services, or Counseling.

Experience/Skills:

  1. Required: Two years of social work, human service, or related professional experience, including demonstrated ability in psychosocial assessment.
  2. Preferred: Bilingual in English and Spanish.

Unless certification, licensure, or registration is required, an equivalent combination of education and experience may be substituted.

Physical Demands and Environmental Conditions

Work is considered sedentary, requiring work indoors in a normal office or patient care environment.
