The Patient Navigator / Community Health Worker is a full-time position located in the Rio Grande Valley - serving the Harlingen & Brownsville, TX area. Flexibility in hours is essential to this role to ensure the needs of the community we serve are being met. Saturday & evening work will be required on a regular basis for health fairs, community events & meetings.
POSITION SUMMARY:
Works closely with the Regional Health Care Director – RGV to facilitate patient navigation and deliver community education for in the RGV. Works outside of the health centers, in various community settings, a majority of the time. Supports marketing and health promotion to increase patient volume and health center revenue. Provides and/or coordinates educational presentations and events in the community. Assists with evaluation of the project. Assists the Habla Con Tu Hermana program team with volunteers as requested.
ESSENTIAL FUNCTIONS:

3.Executes and delivers health promotion initiatives that are designed to increase in-person and telehealth patient volume at RGV clinics, thereby increasing service delivery. Conducts patient navigation activities that help potential clients to access healthcare in person at RGV clinics, and through telehealth at all Planned Parenthood South Texas (PPST) clinics.
4.Develops relationships with organizations in the RGV that serve the client profile of Planned Parenthood and provides staff and clients of those organizations with information about services offered at Planned Parenthood.
5.Works closely with the Regional Health Care Director - RGV, and the Habla Con Tu Hermana program team, to plan and execute various events, information campaigns and sidewalk health fairs at the RGV health centers that promote health center services, achieve project goals and gain positive attention for the organization.
6.Organizes the work of the patient navigation efforts in close coordination with the Regional Health Care Director - RGV, and the Habla Con Tu Hermana program team.
7.Conducts a referral and follow-up system by:
a.Consistently monitoring and reporting referrals from the project’s tracking system.
b.Setting appointments for patients using the EMR database as directed by the Regional Health Care Director - RGV for PPST.
c.Following up on missed appointments in the patient portal according to protocols established by the Quality Assurance & Risk Management Program at PPST and as approved by the Regional Health Care Director – RGV.
8.Provides health education presentations in community on various topics related to family planning, women’s health and sexual health. Such education is culturally competent, scientifically accurate and age appropriate.
9.Assists the Habla Con Tu Hermana Program Manager, in close conjunction with the Regional Health Care Director – RGV, with regular curriculum review to ensure education content is accurate, up-to-date and science-based.
10.As directed, assists with training and coordinating volunteers to represent Planned Parenthood at health fairs and patient navigation efforts.
11.Produces monthly reports, tracking sheets and/or forms for the Regional Health Care Director – RGV and Habla Con Tu Hermana Program Manager.
12.Maintains consistent communication with the Regional Health Care Director - RGV via phone, e-mail or in person.
13.Assists the Regional Health Care Director - RGV with project evaluation.
14.Keeps current regarding the content of sexual and reproductive health issues and the methodologies of community health education.
15.Performs other job-related duties as assigned.
16.Complies with the organization’s House Rules, which are a required part of the organization’s orientation, training and performance standards.
QUALIFICATIONS:
Education:
Community Health Worker (CHW) certification through a state agency or a community college is required.* An Associate’s Degree or higher is preferred. 1-2 years of community education work – as an employee or a volunteer – is preferred. Bilingual (Spanish & English) is required.
*Degrees in the areas of public health, health education, social work, counseling, social/policy sciences, or related fields, combined with at least 2 years community outreach experience can be substituted in lieu of CHW certification. In addition, CHW certification must be obtained within 3 months of hire.
Experience:
Experience in delivering community health education presentations in the areas of family planning and women’s health is highly preferred. Public speaking experience is required. Experience in adolescent issues and advocacy related to women’s health issues and comprehensive sex education is preferred.

·Able to travel regionally and outside the state on occasion.
·Sensitive and competent regarding issues of multiculturalism.
·Comfortable with issues of sexual and reproductive health.
·Able and willing to work for an organization that supports a patient's right to make personal decisions regarding health care including their right to choose what option is best for them if faced with an unintended pregnancy.
