Position Summary

The Patient Navigator is responsible for providing advocacy and case management services for patients, facilitating access to social service benefits and other internal and external resources, and advocating on behalf of the patient. They guide patients through the healthcare system, ensuring access to culturally and linguistically appropriate services in a timely and cost-effective manner. The role includes helping patients arrive prepared for appointments, facilitating communication with healthcare staff, providing educational materials, documenting care plans, and demonstrating cultural competency. The Patient Navigator tracks patients through all diagnostic and treatment plans to ensure timely receipt of care.

Position Details
  • Department: Renal Clinic Med Spec
  • Schedule: Part Time, 20 hrs/wk
  • Education: Minimum of High School diploma or GED; degree in psychology or public health preferred
Requirements and Skills
  • 1-2 years relevant work experience; experience may be waived for candidates with a Bachelor's or higher degree demonstrating proficiency
  • Experience navigating community resources or in healthcare settings preferred
  • Knowledge of community, social, and health resources
  • Empathy and skills in multicultural needs assessments and service planning
  • Ability to establish relationships with patients, families, professionals, and the community
  • Computer proficiency and data entry skills
  • Ability to work in a fast-paced environment with organizational and problem-solving skills
