Job summary

Jasper Grove Recovery is seeking a Patient Navigator to support patients during inpatient treatment and through discharge, coordinating care and connecting clients with community resources for long-term recovery.

The role involves frequent collaboration with treatment teams, external providers, and employers. Schedule is 8:30a-5p SUN-THUR with compensation of $19-$22 per hour. CADC/TCADC certification and a Bachelor's degree are preferred, with at least 1 year in human services.

Qualifications

  • CADC/TCADC certification with a Bachelor's degree preferred.
  • Minimum 1 year of experience in a human service setting with adults or adolescents.
  • Strong commitment to a drug-free lifestyle and responsible alcohol use.
  • Personal interest or experience in substance use or behavioral health recovery strongly preferred.
  • Must meet all requirements outlined by state regulations.

Responsibilities

  • Assess patients' external biopsychosocial needs and develop strategies for addressing them during and after treatment.
  • Facilitate care coordination, aftercare planning, and communication with outside agencies and services.
  • Manage documentation and paperwork related to FMLA, short-term disability, and court matters.
  • Maintain communication with patients' support systems, employers, and referral sources.
  • Keep thorough case notes and update patient records accurately and promptly.
  • Coordinate discharge planning and ensure continuity of care with community-based services.
  • Participate in weekly treatment team meetings to provide progress updates and discharge readiness.
  • Assist with census management by coordinating discharge schedules and communicating updates to clinical and administrative teams.

Position Summary:

The plays a key role for Patient Navigator in supporting patients during inpatient treatment and through discharge by coordinating care, addressing external stressors, and ensuring patients are set up for long-term recovery success. This role involves frequent interaction with patients, treatment teams, external service providers, and community partners.

Schedule: 8:30a-5p SUN-THUR

Compensation: $19-$22

Essential Duties and Responsibilities:

  • Assess patients' external biopsychosocial needs (e.g., housing, financial, legal, medical, employment) and develop strategies to address them during and after treatment

  • Facilitate care coordination, aftercare planning, and communication with outside agencies and services

  • Manage documentation and paperwork related to FMLA, short-term disability, and court matters

  • Maintain communication with patients' support systems, employers, and referral sources

  • Keep thorough case notes and update patient records accurately and promptly
