Job summary

Infinite Medical P.C. is seeking a dedicated LPN CoCM Care Coordinator to join our team in Hollywood, FL. This role focuses on reviewing patient charts and coordinating with the Clinical department to address irregularities as part of chronic care management.

The ideal candidate holds an LPN license and has experience in long-term care, with a preference for behavioral health familiarity to support integrated care plans.

Qualifications

• LPN license required to practice as expected.
• Experience in long-term care preferred.
• Experience in behavioral health preferred.

Responsibilities

• Review patient charts and communicate with Clinical department and providers about irregularities.
• Provide assessment and care management services including validated rating scales and care planning.
• Engage with billing practitioner and maintain registry/tracking sheets.
• Conduct interdisciplinary care plan meetings and coordinate with the care team.

Job Summary

We are seeking a dedicated LPN CoCM Care Coordinator to join our team. In this role, you will be responsible for reviewing patient charts and communicating with the Clinical department and providers about any irregularities as part of chronic care management.

Responsibilities

• Provide assessment and care management services, including:
  • Administration of validated rating scales.
  • Initiation of behavioral health care planning concerning behavioral or psychiatric health problems.
  • Revision and modification of care plans for patients not progressing or whose status changes.
  • Brief psychosocial interventions as needed.
• Engage in ongoing collaboration with the billing practitioner.
• Maintain the registry/tracking sheets.
• Consult with the psychiatric consultant.
• Maintain a continuous relationship with patients.
• Foster collaborative, integrated relationships with the rest of the care team.
• Conduct interdisciplinary care plan meetings to review patient beneficiaries.

Requirements

• LPN License required
• Experience in long-term care is preferred.
• Experience in behavioral health preferred.
