Job summary

HCA Healthcare, Inc. is seeking a Patient Logistics Specialist to help move patients through the care continuum. You will handle intake of referrals, provide status updates with Emergency Departments and inpatient settings, and coordinate admissions or transfers to appropriate units or facilities.

You will respond to phone, fax, and email requests, maintain bed availability, schedule transports, and support discharge planning with a responsive, customer-service mindset.

Qualifications

• 2+ years of healthcare experience preferred.
• Behavioral Health experience preferred.
• Call Center / Transfer Center / Bed Management experience is helpful.

Responsibilities

• Respond to calls with urgency and strong customer service.
• Gather initial information to open new case and prepare clinical information for assessment.
• Coordinate resources to support patient disposition and admissions.
• Maintain active inventory of bed availability for non-HCA bed placements.
• Accept, track and facilitate hospital transfers and transports telephonically and via computer apps.
• Provide status updates to customers on patient case status and bed requests.
• Schedule and prioritize discharge transports and support rapid response to requests.
• Demonstrate clinical decision making to facilitate patient flow through care continuum.

The Patient Logistics Specialist is responsible for the intake of new case referrals including status updates to/from Emergency Departments, Inpatient Hospital Settings, or Community Settings. The Patient Logistics Specialist responds to phone calls, faxes, and e-mail requests, or other forms of intake communication, for clinical information, patient admissions or transfers, transport requests and other non-clinical information.

Primary Duties And Responsibilities

• Responds to phone calls with a sense of urgency and, demonstrating good customer service skills
• Gathers initial information required to open new case/call(s) and prepares clinical information/packet which is to provide to the clinicians for assessment and next steps in advancing patients care.
• Helps to locate available resources and coordinates information to support patient disposition and or admissions to appropriate unit/facility.
• Maintains active inventory of bed availability within the community for non-HCA bed placements and admissions.
• Responsible for accepting, tracking and facilitating hospital transfers and transports telephonically and via computer applications.
• Provides updates to customers on status of patient case(s), i.e. assessment status and bed placement requests
• Assists to schedule and prioritize discharge transports and supports the Patient Logistics Center by assuring rapid response to requests for services
• Demonstrate the ability to process patient acuity utilizing critical thinking skills, clinical decision making, and acquired knowledge to assist in assessing situations and facilitate patient flow through the continuum of care.

Experience

• 2+ years of healthcare experience highly preferred
• Behavioral Health experience highly preferred
• Call Center – Transfer Center – Bed Management experience helpful

Scheduling Requirements

• This position is Part-Time Days and requires a weekend rotation: 12 hour shift requirement unless otherwise noted. This role will require you to work some holidays.

Location Requirements

• You will perform the duties and responsibilities of this position onsite at an HCA Patient Logistics Center (PLC).
• Qualified candidates must live within 60 miles of an HCA PLC in the market area associated with the position.
