Upstream Rehabilitation - Remote Patient Liaison

Upstream Rehabilitation is seeking a REMOTE Patient Liaison (must reside in the United States) to join our team. The Patient Liaison handles patient intake, scheduling, and related tasks to ensure timely and appropriate care.

Responsibilities:
  1. Support scheduling of evaluations and treatments, ensuring timely appointments that meet medical needs.
  2. Review referral sources and documentation to gather necessary information for scheduling.
  3. Use the Patient Liaison mapping system to assign patients to appropriate clinics and specialties.
  4. Record communication preferences with case managers, networks, attorneys, or other entities.
  5. Respond to weekly reports and feedback, reporting progress and issues to supervisors.
  6. Process Functional Capacity Evaluations (FCEs) and other functional testing as directed.
  7. Develop and maintain relationships with clinics, referral sources, and networks.
  8. Perform other duties as assigned.
Qualifications:
  • High school diploma or equivalent.
  • Experience in data entry and managing incoming calls.
  • Strong relationship-building and interpersonal skills.
  • Ability to multitask and problem-solve, including conflict resolution.
  • Familiarity with Upstream's services, including physical and occupational therapy, dry needling, etc.
  • Proficiency with computer platforms including EMR, Microsoft Office, and web portals.
  • Excellent communication skills, both oral and written.
