Description

The Patient Navigator coordinates services and guides patients across the continuum of care to help achieve the best possible quality, experience and affordability outcomes. The Patient Navigator works as part of an integrated care team that includes providers, clinic staff, social workers, financial counselors, community agencies and educators to effectively address the needs of patients in a collaborative, patient-centered environment. The Patient Navigator has a solid understanding of the various Providence-provided and community-provided resources that relate specifically to their patient population and can advise patients on how to best take advantage of these resources.

Required Qualifications:
  • 3 years' experience in a medical clinic, office setting or administrative setting.
Preferred Qualifications:
  • Bachelor's Degree.
  • Education in a health-related field, e.g., public health, sociology, psychology or health education or 5 years' work experience as Medical Office SSC or Coordinator.
  • Upon hire: Certification in an approved program of phlebotomy or medical assisting (for Postpartum Care Center, Mother Baby Clinic PPMC).
  • 2 years' experience in the appropriate specialty setting.
  • Experience facilitating large group sessions and/or discussions.
