Qualifications

• License in good standing to practice Optometry in Indiana required.
• National Provider Identification (NPI) required.
• TPA License required.
• Ability to utilize necessary equipment and move about the office.

Responsibilities

• Evaluate patient’s vision and health care needs.
• Document findings in patient records.
• Consult with patients regarding vision care needs.
