You will educate patients, coordinate programs, and foster lasting engagement, delivering a high-quality, empathetic patient experience.

The role emphasizes clear communication, proactive follow-up, and collaboration with clinical teams to optimize outcomes and satisfaction for every patient.

Responsibilities

• Guide patients through onboarding, treatment progression, and ongoing care.
• Educate patients on treatment options, timelines, and expectations.
• Ensure patients remain engaged and consistent with their care plans.
• Celebrate patient progress and reinforce long-term success.
• Build strong, trust-based relationships with patients.
• Address patient questions, concerns, and barriers to adherence.
• Provide clear, simple, and proactive communication at every stage.
• Ensure patients feel supported, informed, and confident in their care.
• Manage patient accounts, subscriptions, and documentation.
• Coordinate with clinical and administrative teams to ensure seamless care.
• Support medication ordering, tracking, and follow-up.
• Monitor patient activity and re-engage as needed.
• Partner with PRCs and clinical staff to ensure a consistent patient experience.
• Support clinic flow when needed to maintain efficiency and patient satisfaction.
• Communicate effectively across teams to ensure alignment and continuity of care.
