What to Expect (Job Responsibilities):
- Reach out to patients after a recent hospital visit and connect them with a registered nurse
- Obtain basic information regarding patients' recent hospital stays
- Review and prioritize patient census for effective outreach

- Address inquiries and schedule initial and annual appointments
What is Required (Qualifications):
- High School diploma or equivalent
- Minimum of 1 year of experience in a call center, sales, healthcare, or customer-focused environment

- Proficiency in entering documentation into designated systems
