General Job Description

Under general supervision, the Patient Engagement Specialist I provides patient engagement services to recruit patients into the program's system of care. The role involves supporting the planning, development, implementation, and evaluation of services in line with contractual and departmental guidelines. The focus is on connecting the medically underserved to community-based healthcare services, with specific tasks varying based on program, grant, and patient demographics.

Job Responsibilities

  • Develops, identifies, and provides referrals within and outside of FHCSD, and monitors these referrals effectively.
  • Communicates effectively to recruit new patients into the system of care, engaging well with community members and patients from diverse backgrounds.
  • Leverages community resources to raise awareness of FHCSD, maintains good relationships with community partners, and uses these resources to connect patients with FHCSD services.
  • Works collaboratively as a productive and collegial team member.
  • Participates in planning, developing, executing, and evaluating patient engagement strategies. Investigates leads for recruitment and engagement opportunities.
  • Performs additional duties as assigned.
  • Produces accurate and timely written work (plans, flyers, reports) and utilizes computer resources effectively to meet organizational needs.

Qualifications

  • Ability and means to travel within San Diego County, including areas with limited public transportation; proof of liability and property damage insurance on the vehicle used is required. Driver requirements include being at least 21 years old, licensed for a minimum of 3 years, with no violations or accidents within the past 3 years, and no suspensions or felony violations related to driving within 7 years.
  • At least 1 year of experience in sales, patient engagement, community education, community health, or outreach programs; relevant experience in the specific program area may be required.
  • Associate's Degree in psychology, sociology, social work, communication, public health, or related field is strongly preferred, or relevant work/life experience may substitute.
  • High School diploma or GED required, or an equivalent combination of education and experience that enables the performance of essential duties and meets certification requirements.

Skills and Experience

  • Strong interpersonal skills to interact effectively with clients and co-workers in a diverse, multicultural environment.
  • Ability to multi-task, adapt to changing priorities, ensure accuracy, and work efficiently in a fast-paced setting.
  • Basic computer literacy, including experience with databases and Microsoft Office Suite.
  • Bilingual abilities in English and Farsi, Dari, or Pashto are preferred.
  • Excellent communication skills, including active listening and paraphrasing.
  • Intermediate skills in investigating leads for patient recruitment and engagement.
  • Knowledge of community resources and health/social service systems in San Diego County, with the ability to establish and maintain community partnerships.
  • Knowledge of current health science and practices related to the program's focus area.

Compensation

Base salary range: $23.00 - $27.46 per hour. Final salary will be determined based on location, skills, education, and experience. We value pay equity and internal consistency.
