What to Expect (Job Responsibilities):
- Conduct patient outreach through telephonic and in-person home visits to foster engagement in the care model
- Document patient interactions in electronic medical records while maintaining HIPAA standards
- Communicate complex topics among all parties involved in patient care, including family members and medical providers
- Partner with patients and their caregivers to identify goals and address barriers to care
- Foster relationships with patients and collaborate with Community Health Workers, Social Workers, and clinicians

What is Required (Qualifications):
- Experience in healthcare-related engagement, sales, and/or marketing
- Strong verbal and written communication skills
- Proficient in technology, including Excel and EHR/EMR platforms
- Understanding of Medicare and Medicare Advantage programs
- Access to reliable transportation for travel within assigned territories

How to Stand Out (Preferred Qualifications):
- Fluency in Spanish (written and oral)
- A demonstrated ability to operate independently in a fast-paced environment
