OhioHealth seeks a registered nurse to lead patient education and coordination of care across inpatient and outpatient settings. The role focuses on disease management education, facilitating seamless transitions, and promoting patient engagement in their care plan.
The RN will assess learning needs, provide tailored education, document care, and collaborate with the Nursing Research Excellence team to improve outcomes.
Assure the patient and/or his/her family are provided with education that can enhance their knowledge, skills, attitudes and behaviors necessary to fully benefit from the health care interventions provided. To ensure that the patient/family are encouraged to participate in an appropriate out-patient program and that a seamless transition can be achieved to an out- patient program or other community resources, promoting self responsibility. This role was created to provide specialized knowledge of the disease process related to patient education and outcomes among leadership, staff and physicians.
