Provide comprehensive, health education and resource information details and support for patients, their families, Sutter Staff (as needed) and patrons in the local community. Delivery modalities include both virtual and in person for our department’s individual and group services. Provide an on-site presence and assist patrons with health education and resource needs at designated Health Education Resource Centers (includes helping Walk-in Patrons, addressing patient needs via phone calls, My Health Online, and email follow up). Regularly review and follow up on Provider generated Patient Referrals via EPIC. Investigate and provide individualized patient information, and resource needs via My Health Online messaging, telephone conversations, and scheduled in-person visits. Engage patients/patrons in motivational coaching sessions aimed at supporting development of personal reasons for healthy lifestyle changes (which may include Provider prescribed or recommended treatments) and support behavior change skills and effective goal setting. Evaluate requests for materials from a variety of evidence-based sources and gather quality information for distribution. Assist with virtual webinar/class and group facilitation. Design new class/group offerings with team, as needed. Participate in collaborative projects with department workgroups, including clinical areas. Present Health Education Department Services and offerings to Providers, staff, and a variety of department groups and community organizations. Tally patient/patron engagements, review and use data as a tool for setting team goals. Pay Range is $44.97 to $58.46 / hourThe salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
