Job summary

Community Medical Centers in Stockton, CA is seeking a Substance Use Counselor II to support patients through counseling and recovery planning within a multidisciplinary team.

You will conduct assessments, coordinate referrals, and deliver trauma-informed care, while helping patients navigate housing, benefits, and transportation to access needed services. The role emphasizes motivational interviewing and community engagement.

Qualifications

• High School Diploma or GED required.
• One year of experience as a Substance Use Counselor.
• Alcohol and Drug Counselor Certificate issued by the State of California.
• Knowledge of chemical dependency and substance use dynamics.
• Excellent Microsoft Office skills.
• Valid California Driver’s License and a current/obtainable BLS certification.

Responsibilities

• Conduct individual counseling related to substance use and recovery.
• Transport patients to/off hospital grounds for care activities.
• Perform psychosocial histories via patient and family interviews.
• Collaborate with treatment team to plan and evaluate approaches.
• Document progress and coordinate referrals to community resources.
