Job summary

Auburn Nursing & Rehabilitation Center is looking for a dedicated Speech Language Pathologist to join its skilled therapy team. The SLP will assess and treat residents with speech, language, cognitive, and swallowing disorders, developing individualized plans to improve independence and quality of life.

The ideal candidate will have a Master’s in Speech Language Pathology, Kentucky licensure, and ideally CCC-SLP credentials, with strong communication and documentation skills and a commitment to

Qualifications

• Master’s degree in Speech Language Pathology from an accredited institution.
• Current, valid Kentucky license in Speech Language Pathology.
• CCC-SLP from ASHA preferred.
• Strong communication, documentation, and organizational skills.
• Compassionate, dependable, and dedicated to improving residents’ quality of life.

Responsibilities

• Evaluate residents with speech, language, voice, cognitive, or swallowing impairments and develop individualized treatment plans.
• Provide direct therapy services in accordance with each resident’s care plan and physician orders.
• Assess resident progress and adjust treatment plans as necessary to achieve optimal outcomes.
• Collaborate with nursing, therapy, and dietary staff to support residents with communication or swallowing needs.
• Educate residents, families, and caregivers about communication strategies and compensatory techniques.
• Document therapy sessions, progress notes, and treatment summaries in compliance with facility policies and state/federal regulations.
• Participate in interdisciplinary care planning and discharge planning processes.
• Maintain therapy equipment and ensure a safe, supportive environment for residents.
