Job summary

Live Well Homecare is seeking a registered nurse to provide patient-centered nursing care using the full nursing process—assessment, diagnosis, planning, implementation and evaluation—while partnering with patients and families for personalized plans.

You will collaborate with interdisciplinary teams, mentor colleagues, help develop protocols and participate in data collection to drive quality improvement, all while ensuring compliance with safety standards and regulatory requirements.

Qualifications

• RN license in good standing (single or multi-state) with BLS and ACLS certifications.
• Associate's or Bachelor's degree in Nursing.
• Minimum 2 years of clinical experience and computer proficiency; Epic and 3M experience preferred.

Responsibilities

• Coordinates safe, high-quality care through effective communication, documentation, and collaboration within interdisciplinary teams.
• Supports performance improvement initiatives, research, and evidence-based practice to enhance patient outcomes.
• Preceptor or mentor to foster professional growth and a healthy work environment.
• Develops protocols, participates in data collection, and promotes efficient resource use.
• Ensures compliance with organizational policies, safety standards, and regulatory requirements.
