Job summary

Jefferson Health is seeking a dedicated professional nurse to provide patient- and family-centered care using the nursing process. You will develop care plans with patients and designated care partners, addressing physical, psychological, emotional, and social needs in a diverse patient population.

Shifts are 7:00 am to 7:30 pm, three days a week (0.9 FTE) with on-call requirements. You will collaborate with inter-professional teams to ensure safe, compassionate, and culturally sensitive care

Qualifications

• Specialized Diploma in Nursing from an accredited program.
• Strong patient- and family-centered care approach is required.
• Excellent communication and collaboration with patients, families, and care partners.

Responsibilities

• Utilize nursing process to plan and deliver patient care.
• Collaborate with patients and families to identify learning needs and preferences.
• Adhere to nursing practices, protocols, and ethical standards to minimize risk.
• Use data and documentation to monitor care outcomes and support quality improvement.
